Benzoyl Peroxide: Acne Treatment & Product Guide
Benzoyl peroxide (BPO) is a first-line topical acne treatment. It kills Cutibacterium acnes, helps unclog pores, and has some anti-inflammatory effect. Unlike topical antibiotics, bacteria do not develop resistance to it.
It is available OTC and by prescription in washes, gels, creams, and lotions, usually at 2.5%, 5%, or 10%. Higher strength is not clearly more effective and is more irritating. For leave-on products, 2.5% often matches 5–10% on lesion reduction with less dryness. For short-contact washes, 4–5% is a common practical range.
Results typically start around 4 weeks with consistent use. Combination products (BPO + adapalene, or BPO + a topical antibiotic) are often more effective than BPO alone for inflammatory acne.

- May help limit scarring by clearing inflammatory lesions faster. Studies of BPO or BPO + adapalene have shown better improvement in both active lesions and scars than no treatment.
- Useful on body acne (chest, back, shoulders) where thicker skin often tolerates higher strengths or wash-off formulas better than the face.
How to use it
- Start low and slow: thin layer, once daily or every other day, then increase if tolerated.
- Apply to clean, dry skin. Leave-on gels work harder than rinses; washes need ~30–60 seconds of contact.
- Follow with a non-comedogenic moisturizer. Use SPF in the morning — BPO increases photosensitivity.
- It bleaches towels, pillowcases, and dark clothing. Let it dry fully before dressing; use white linens if possible.
Common side effects: dryness, redness, peeling, stinging. These often settle after 2–4 weeks (“hardening”). If you get swelling, severe rash, or breathing issues, stop and get medical help — true allergy is uncommon but possible.
